OBJECTIVE: To see the prevalence of lower lung field tuberculosis in diabetics and non diabetics by radiograph.
DESIGN: Comparative case control prospective study.
PLACE AND DURATION OF STUDY: People’s Medical College Hospital Nawabshah, during the period of one year from 1st January 2008 to 31st December 2008.
MATERIAL & METHODS: All the adult patients new cases (Previously untreated) sputum smear AFB positive pulmonary tuberculosis were selected from chest ward and OPD, who had lesion below an arbitrary line across the hila in their chest x-rays were included in this study as case of lower lung field tuberculosis. Blood sugar fasting and random were performed in each patient. Diagnostic value of fasting plasma glucose concentration is 7.0 m mol/l (126 mg/dl) and above.
RESULTS: Out of a total 80 pulmonary tuberculosis patients 26 patients (32.50%) with diabetes, as compared to 6 patients (7.5%) in the non-diabetic patients had lower lung field tuberculosis (LLFT). It was more common in males 13 (16.25%) V/S 4 (5%) than in females 7 (8.75%) V/ S 2 (2.5%) patients had lower lung field tuberculosis (LLFT). Majority of patients 12 (15%) were in 41 – 60 years of age group. Unilateral disease was more common 68 (85%) V/S 52 (65%) and the right side was more frequently affected and cavitation 16 (20%) V/S 20 (25%) and nodular opacities 48 (60%) were the main radiological findings where as consolidation more common in PTB patients 10 (12.5%) V/S PTB DM 8 (10%) PTB with diabetic patients had significantly higher incidence as compared to non-diabetic patients had lower lung field tuberculosis (LLFT).
CONCLUSION: Lower lung field tuberculosis is fairly common in diabetic as compare to non diabetic PTB patients. Diabetic mellitus increase the risk of lower lung field tuberculosis.
