OBJECT: To evaluate the diagnostic value of ultrasound guided transthoracic biopsy in peripheral lung and mediasteneal masses with trucut needle.

DESIGN: A prospective study was conductive between January 2007 to October 2009 in the department of Pulmonary Chandka Medical College Hospita & Larkana and Peoples Medical College Nawabshah .

PATIENTS AND METHODS: There were 72 patients, All the patients had peripheral/mediastinal lesions adjacent to chest wall with accessible to ultrasound window, had normal platelets count, and prothrombin time were included in this study. The biopsies were performed with trucut needle having 16 gauge.

RESULT: A definative histological diagnosis were made in 66(91.67%) patients. The remaining 06(8.33%) were non-diagnostic. Out of 66 patients, 63(95.45%) had malignant lesions and 03(4.55%) were benign. Among the malignant lesion 30(47.62%) squamous cell carcinoma, 13(20.63%) adenocarcinoma, 10(15.87%) small cell carcinoma, 06(9.52%) large cell carcinoma, 01(1.59%) pulmonary blastoma, 1(1.59%) malignant mesothelioma, and 1(1.59%) had non hodgkins lymphoma. The three benign lesions consist of chronic inflammation.
