OBJECTIVE : To determine the diagnosis accuracy of poly cystic ovarian syndrome (PCOS) predominantly on clinical ground and metformin response on menstrual irregularities and conception rate
STUDY DESIGN: Quasi experimental study
PLACE AND DURATION OF STUDY: Sheikh Zyed women Hospital Larkana from September 2007 to august 2008.
PATIENTS AND METHEDS: Women fulfilling the inclusion criteria (oligo/hypomenorrhea,Infertility,wait gain,hyper androgenism)were enrolled, presence of 8 or more follicles arranged in peripheral without mature follicles is cut off number for positive ultrasound. Prolactin and thyroid profile were performed and patients with abnormal levels from study. Metformin was started with a dose of 500mg thrice daily for 6 months and 6 months later evaluated for response to metformin and those who failed to conceive were given clomiphene citrate along with metformin. Again fertility was re evaluated at the 10 months.
RESULTS: At the start of study 81% women had menstrual irregularities and 84% had infertility. Obesity was seen in 62% of patients while hirsutism was present in 72%. On ultrasound 90% women had evidence of poly cystic ovaries. After 6 months of metformin therapy 80% patients had achieved correction in their menstrual irregularities. After 6 months of metformin therapy alone 50% patients conceived while additional 23% conceived both on metformin and clomiphene citrate during next 4 months. Over all fertility rate was 73% at the end of 1 year. There was statistically significant change in pre treatment and post treatment BMI.
CONCLUSION: The combine clinical features( menstrual irregularities, infertility, weight gain ,and hirsutism)provide an good and appropriate criteria for clinical diagnosis of PCOS. Metformin alone was also effective treatment for PCOS patients.
