OBJECTIVE: To determine various indications for vaginal hysterectomy and consequent complications at Liaquat University Hospital, Jamshoro.

PATIENTS AND METHOD:  This observational descriptive study was conducted in the Department of Gynecology & Obstetric Unit I, Liaquat University Hospital Hyderabad from 2004 to February 2009. A total of 552 cases of vaginal hysterectomy with or without repair performed for benign etiology were assessed.

RESULTS: Age ranged from 26 -69 years with mean age of 48 years, Uterovaginal Prolapse was the most common indication found in 282 patients (51.08%). Second most common indication was Dysfunctional uterine bleeding observed in 192 patients (34.78%). overall incidence of major complications was 4%. Incidental bladder opening took place in 6 patients (1.08%). Four patients (0.72%) had primary hemorrhage, while 2 patients (0.36%) had secondary hemorrhage. Primary hemorrhage was observed in 2 cases of plain vaginal hysterectomy and in 1 case of vaginal hysterectomy with repair. Three patients had secondary hemorrhage, secondary to plain vaginal hysterectomy in one patient and vaginal hysterectomy with repair in two patients. A total of 14 cases (2.53%) with vault sepsis were managed with systemic and local application of antibiotics and drainage. Four patients (0.72%) couldn’t survive. In all these patients cause of death was sepsis.

CONCLUSION: Vaginal hysterectomy provides considerable benefits to patients, however by carrying out careful patient selection, proper and thorough pre-operative assessment, expert surgical techniques and vigilant post operative care complication rate can further reduce down.
